Bifsha attributes part of his progress to years of refining his business model. He previously built multiple chiropractic and medical practices in Florida, developing systems designed to run efficiently without excessive overhead.
“Most medical franchises start at $270,000 to $400,000,” he notes. “We’ve worked to structure Spine Labs to be fully operational for under $200,000, potentially making it a more accessible investment for professionals looking to enter the healthcare space.”
No Insurance? No Problem
Unlike many traditional chiropractic offices, Spine Labs does not accept insurance. Bifsha explains that the decision was strategic.
“Insurance billing can be challenging,” he says. “Reimbursement rates fluctuate, vary by state, and sometimes delay care.”
By removing reliance on insurance, Spine Labs strives to maintain lower, more predictable costs. This way, patients know upfront what they’re paying for.
This direct-pay model has seen success in other healthcare sectors, but it’s still emerging in chiropractic care. Spine Labs is among the early franchises to adopt this approach at scale.

Every new patient at Spine Labs receives a comprehensive assessment, often followed by their first treatment on the same day. Patients also receive a customized physical therapy plan with step-by-step exercise videos.
“Our goal isn’t to push unnecessary treatments,” Bifsha emphasizes. “We share the facts, let patients experience the care, and allow them to make informed decisions.”
