Philadelphia, PA – Patient Care Coordination Inc. (PCC) is celebrating its 10th anniversary, marking a decade of significant achievements in service coordination and healthcare management under the leadership of its Founder and Executive Director, Paul S. Rimar. Established in 2014, PCC has grown from a small office in Philadelphia to a central business hub that now serves over 6,500 participants across 21 counties in Pennsylvania.
Paul Rimar’s journey in founding PCC is deeply rooted in his extensive background in social work and managed care. With a Master’s in Social Work and a Certificate in Gerontology from Temple University, Rimar leveraged his diverse expertise in long and short-term care, acute care hospital settings, hospice and palliative care, managed care, and nephrology and transplantation. His vision for PCC was centered on achieving the healthcare “triple aim”: improving the patient experience, enhancing population health, and reducing healthcare costs.
Starting as the Executive Director, Service Coordinator, Administrative Assistant, and Marketer, Rimar’s hands-on approach and unwavering commitment to excellence have been integral to PCC’s success. His dedication to recruiting a passionate and skilled team has allowed PCC to grow rapidly, outgrowing its initial office space within a year and moving to a more central location to better serve its participants.
PCC places a strong emphasis on serving waiver-eligible participants with integrity, dignity, and compassion. The agency’s growth is a testament to its effective utilization management and its substantial impact on value-based care and improved health outcomes throughout Pennsylvania. As a payer of last resort, PCC plays a crucial role in enhancing the health of local populations, aligning with its mission to serve the community’s vulnerable members.
One of the distinguishing features of PCC is its dedicated focus on LGBTQ initiatives and partnerships. Rimar, who personally understands the challenges faced by the LGBTQ community, has embedded these values into PCC’s operations. This focus is reflected in PCC’s inclusive employment policies, active community engagement, and tailored patient care for LGBTQ participants. The organization has formed partnerships with numerous LGBTQ community events and organizations, including Code RED, Mazzoni Center, and Philly AIDS Fund, and maintains an active LGBTQ committee to further this mission.
PCC’s commitment to value-based care extends to its partnerships with community health organizations. These collaborations ensure greater continuity of care by bridging the gap between acute care systems and community-based resources. PCC’s approach highlights the importance of proper assessments and individualized care plans to meet participants’ needs, significantly contributing to better population health outcomes.
Throughout its decade of operation, PCC has been recognized for its excellence and integrity. The organization has received several accolades, including the 2018 Business Integrity Award, the Philly 100 Award for consecutive years, and inclusion in the Inc. 5000 list in 2019 and 2020. Most recently, PCC was named one of the Premier Places to Work in 2023.
Reflecting on the journey, Paul Rimar expresses profound gratitude for the support and collaboration within the PCC team and the wider community. “Meeting such a wonderful group of professionals has been a true blessing. Learning from such amazing leaders has contributed to the professional I have become,” says Rimar. His leadership and vision have not only driven PCC’s growth but have also fostered a culture of inclusivity and community engagement.
As PCC looks to the future, it remains steadfast in its mission of providing high-quality care coordination and enhancing the health and well-being of its participants. The organization’s ongoing efforts to advocate for and support the LGBTQ community, along with its commitment to value-based care, position PCC as a leader in the service coordination industry.
Looking ahead, PCC is set to continue its trajectory of growth and community involvement. The agency plans to expand its services and further strengthen its partnerships with local health systems and community organizations. By doing so, PCC aims to enhance its capacity to provide comprehensive and compassionate care to its participants.
PCC’s story is one of resilience, dedication, and community service. Under the visionary leadership of Paul Rimar, the organization has made a lasting impact on the lives of countless individuals and has set a benchmark for excellence in service coordination. As PCC celebrates this milestone, it remains committed to its core values and looks forward to a future filled with continued success and community enrichment.
